At Roots Wellness Center, we provide comprehensive DC:0–5™ Early Childhood Mental Health Assessments for infants, toddlers, and young children ages 0–5. Our evaluations help families understand their child’s emotional, social, developmental, and relational needs through a trauma-informed, culturally rooted, and family-centered lens.
Our licensed clinicians take time to learn each child’s story, strengths, and environment. We collaborate closely with caregivers to ensure the assessment process feels supportive, gentle, and empowering—never clinical or overwhelming.
What Our Assessments Include:
- A full DC:0–5™ diagnostic evaluation
- Family and caregiver interviews
- Observation of play, behavior, and parent–child interaction
- Screening for trauma, development, attachment, and emotional well-being
- Review of medical, developmental, and educational history
- Culturally attuned interpretation of symptoms and behaviors
What Families Receive:
- A written DC:0–5™ report
- Clear, practical recommendations
- Referrals to supportive services such as therapy, early childhood programs, parenting support, CTSS, day treatment, or community resources
- Optional care coordination if families would like help getting connected to services
