Children’s Mental Health Targeted Case Management

At Roots Wellness Center, we offer specialized Children’s Mental Health Targeted Case Management (TCM) services to support children and youth navigating behavioral, emotional, and mental health challenges. We ensure families can access the right services at the right time — through compassionate, culturally rooted, and trauma-informed care coordination.

What We Provide

• Comprehensive assessment & care planning — We begin with a detailed evaluation of each child’s mental health, social, educational, and family needs.
• Individualized care coordination — Our case managers partner with families to develop a personalized plan that reflects their goals, strengths, and cultural values.
• Referrals and linkages — We connect families to therapy, psychiatry, school supports, CTSS, IOP, early intervention, and community resources.
• Ongoing monitoring & support — We provide regular check-ins, adjust plans as needs evolve, and ensure families remain connected to ongoing supports.
• Family advocacy — We help families navigate complex systems — schools, providers, social services, housing, and county programs — while advocating for their child’s needs.

How to Start Services

If you have a current Diagnostic Assessment (DA), please send it to info@rwc-mn.com.
If you need a new DA, you can make a referral online, and we will assign a clinician to complete one.
